Note: If Adjustable Rate is selected in the Amortization Type field, the following fields without a drop-down list will be displayed: Current Index Value, ARM Index Margin, ARM Qualifying Rate, Initial Period Prior to First Adjustment, and Subsequent Adjustment Period.
